You're flushing it. You've made your pass, you think you're in the portal vein, before you hook up the contrast and try and aspirate, you just take some saline. If you do contrast, you start getting blobs of contrast everywhere. You just do saline, just make sure you can flush forward just a little bit with saline, and then hook up your 10 cc contract syringe and start backing up while you're aspirating.
